YOU MUST COMPLETE THE FOLLOWING PRIOR TO GETTING ON THE ICE

  2. Mid-West Amateur Hockey Association (MWAHA) Background Screening - Contact Monte Ames to request a consent to background screen. Monte can be reached at 641-420-5759.
